jQwery Datepicker Yoko theme BE 2.5 help

I would like to know how to implement this code in this form for the textbox Date, I am not sure where to put the code or how to hook it to the textbox. I have several forms I would like to use the datepicker on.

I would like the date format to be DD, d MM, yy

Hi jerrysdean,
I had take a look on your source code and I didn't find a reference to jquery-ui.js .
So, you have to edit your site.master file theme and add this line
<script src="http://code.jquery.com/ui/1.10.3/jquery-ui.js"></script>
<script>
  $(function() {
    $( "#ctl00_cphBody_Date" ).datepicker();
  });
</script>
after jquery reference, to select your input element.
